We will be hosting a walk-in SCIS clinic for registration applications for SCIS cards. This clinic is for applications only. If you are looking to renew your card, please book an appointment.

What to Bring to your Appointment

• Valid ID (Any government issued photo ID such as Drivers License / Identification Card)

• Long form Birth Certificate — must have both parents names (if applicable)

• 2 identical passport style photos

• Any or all supporting ancestry documentation

• A friend to confirm your identity
